obtain the form; click Secure
Order Form. You will be sent a PDF file. To receive
it, you will need Adobe Acrobat Reader. If you do not have Acrobat
Reader you can download it for free by clicking here
or by clicking on the "Get Acrobat Reader" button.
•Enter all of the information on the form.
•The form will tabulate the quantity and price columns to give you the total costs.
•Delivery charges for the U.S. and Canada can be added simply by clicking the appropriate button. For our friends from other countries, we will calculate your delivery charges and send you a return email. We will then ask you to reply back to us before we process your order.
•When the form has calculated the total payment due, click the PURCHASE button. Your order will be sent to our office. We will send you a confirmation that we have received your order, and begin processing immediately.
